GKV Reform 2027: Everything That Changes for Statutory Health Insurance in Germany

Germany’s statutory health insurance system (GKV) is undergoing its most significant structural reform in years. The changes taking effect in 2027 — and some already from mid-2026 — will require millions of GKV members to pay significantly more out-of-pocket for everyday medical care.

Here is a complete breakdown of every change, the financial impact, and what options remain open for high earners eligible for Private Health Insurance (PKV).

The Two JAEG Thresholds for 2027: A Critical Deadline

Perhaps the most consequential change for people considering a switch to private health insurance is the introduction of two separate JAEG thresholds in 2027:

JAEG TypeFor PKV StartEstimated 2027 Amount
Regular JAEGPKV start on or before 31.12.2026approx. €81,000/year
Extraordinary elevated JAEG (+€3,600)PKV start from 01.01.2027 onwardsapprox. €84,600/year

What this means: Anyone who starts PKV coverage in 2026 (by 31.12.2026) only needs to meet the regular threshold (approx. €81,000). Those starting from 01.01.2027 face the higher extraordinary threshold (approx. €84,600).

The Deadline That Matters

To switch from GKV to PKV with an effective date of 01.12.2026, you must cancel your GKV membership by 30.09.2026 at the latest. This deadline has now passed. If you have not yet acted, the next opportunity means qualifying under the higher 2027 threshold (approx. €84,600).

⚠️ Official final confirmation of both threshold amounts is still pending from German authorities (Stand September 2026, source: ARAG Krankenversicherungs-AG).

Co-payment Increases: You Pay More for Every Treatment

Starting 01.01.2027, statutory co-payments across multiple treatment areas are being substantially increased:

Treatment AreaCurrent Co-paymentCo-payment from 2027
Medications€5.00 – €10.00€7.50 – €15.00
Physiotherapy / Home Nursing Care€10.00 per prescription€15.00 per prescription
Hospital Stay (up to 28 days)28 × €10 = max €28028 × €15 = max €420

A GKV member with a chronic condition requiring regular physiotherapy could easily face €200–€500 in additional annual out-of-pocket costs from 2027 onwards.

Dental Subsidy Cuts: Your Fixed Bonuses Drop

GKV dental fixed subsidies (Festzuschüsse) are being reduced across all categories:

Dental Checkup HistoryCurrent SubsidySubsidy from 2027
No documented checkup history60%50%
Continuous checkups for 5 years70%60%
Continuous checkups for 10 years75%65%

This means a patient who faithfully attended all checkups for 10 years will still see their dental fixed subsidy drop by 10 percentage points from 2027.

Homeopathy: No Longer Covered from 01.01.2027

Since 30.07.2026, homeopathic and anthroposophic medicines can no longer be prescribed at GKV expense. Health insurers could voluntarily cover costs until end of 2026. From 01.01.2027, this voluntary option also ends entirely.

Family Insurance Restriction (from 2028)

Starting 2028, the previously contribution-free family co-insurance (beitragsfreie Familienversicherung) will be restricted for spouses and registered civil partners. An additional contribution may become payable for these family members.

Exception: Children remain contribution-free under family insurance.

BBG and JAEG: Above-inflation Increases

Both the Beitragsbemessungsgrenze (BBG / assessment ceiling) and the Jahresarbeitsentgeltgrenze (JAEG / income threshold) will be increased beyond normal annual adjustments. This directly means:

  • Higher GKV maximum contributions for high earners
  • Higher employer PKV subsidy ceiling for privately insured employees (the employer PKV co-payment is capped at 50% of the maximum GKV contribution, which rises with BBG increases)
  • Narrower window for new GKV entrants at the standard threshold

What This Means for PKV-Eligible High Earners

The 2027 GKV reform accelerates the financial case for switching to private health insurance:

  1. Higher co-payments mean GKV becomes progressively more expensive at point-of-service.
  2. Lower dental subsidies widen the coverage gap that PKV dental tariffs fill completely.
  3. The extraordinary JAEG creates a permanent new threshold gap for anyone who delays beyond 2026.
  4. Family insurance restrictions (2028) remove one of GKV’s historically strongest advantages for families with one non-working spouse.

Private health insurance premiums are calculated at entry (age + health), not on income percentage — and remain contractually fixed in their benefit scope regardless of government reform cycles.
